Are you able to support individuals to obtain housing, property or a tenancy?

FHC has a housing partner who has supported more than 100 of our service users access a quality and affordable housing solution. Where required work together with our housing partner to put together a package of support and housing that are exclusive of one another. This support can start right from searching fro the right property and continues right through to moving in to the new home.
 
Do you support people with complex health needs?

Whilst our principle focus is learning disabilities we recognise that many who use our service may also have complex health needs. We support a high number of people who have complex physical disabilities, those with a mental health diagnosis and those who may have a sensory impairment. We also support those who require intravenous feeding (PEG feeding solutions etc).
 
What is your experience of supporting people with Autism and Aspergers Syndrome?

We have, and continue to, support a number of autistic adults and those who diagnosed with Aspergers Syndrome. FHC is proud of its successes in supporting people who’s autism has a profound effect on his/her life. Our support packages are individually tailored to ensure that we fully understand the individual in areas of his/her life and to support the individual; to reach his/her full potential despite the obstacles presented by autism.

 
Do you support young adults in transitional services?

FHC recognises the specific challenges that learning disabled young people face when moving from children’s services and moving into adult services. We recognise the need to support younger adults in a way that helps make this transition a positive experience. Our positive approach to partnership working and engaging with the various agencies both in children’s and adult services has enabled us to successfully support a number of young adults in to adulthood and develop a sound platform in all areas of life including education, work, housing and leisure.

 
Do you support people who have an individualised budget?

Yes. FHC has a number of systems that include finance and personal contracting in place. These systems support all those who may have an individualised budget, a direct payment and/or individual service funds. We have a growing number of people using our service who take advantage of such systems.
 
I would like to talk to you about an individual / group of individuals who require a service; I am unsure of the best solution to meet ther needs. Can I discuss this with you? Will there be a cost for this?

We are happy to discuss any potential opportunity to support individuals to achieve their desired support and/or housing package. As all of our services are bespoke we enjoy sitting down with people, understanding the requirements and then coming up with some options that we feel will best meet the need. We like to be creative and can use a number of models to build the required support and/or housing package. This could also include developing an individual model of support that we may not currently widely offer. This comes at no cost to the individual or the local authority. Our assessment time, meetings and discussion time and our full written proposals are free of charge and come with no obligation to any party. You can contact us via the usual referral route to discuss your requirements.

 
Are you open to undertaking work on a ‘Spot Contract’ basis

If we do not have an appropriate contract within your area or our local contract does not cover the requirements of the service/s you require from us then we are more than happy to discuss spot contracting arrangements with you.

 
Are you able to support people who’s funding comes from a number of sources?

Yes. We have lots of experience of supporting those who’s funding is made up from a number of sources. This includes local authorities, health authorities, supporting people, independent living fund, personal finance and/or funds held in trust.

 
We have a number of people currently living in residential care who want to move in to their own homes... how can you help?

We can help in a number of ways. Firstly if housing is required we are able to work with our housing partner to look at options available. Secondly we have supported the closure of a number of NHs and Local Authority care homes. This has included taking on the full management of the home and working closely with the local authority and individuals to close the home to resettle people into their new homes; providing support staff to the home to manage the transition process and putting together new housing and support packages to enable the individuals to safely and securely leave the residential service and begin life in his/her new home and support package.

 
I support an individual who lives out of the area but wants to live closer to home... can you help?

Yes we have supported a number of people to return locally to be closer to family and friends. We are able to carry out our assessments and transitional work by visiting and spending time with the individual in their current home; distance is not a concern. Our staff and services are fully mobile.
