How to access Future Home Care services:
By Referral
Referrals are usually made by the individual's social worker but anybody is able to make a referral including GP, community nurse, health professional, advocate, family member or the individual can refer him/herself. A simple referral can be sent to you in the post, via fax or on email.

By Assessment
When we receive a referral we usually arrange to visit the individual with somebody familiar to that person. In most cases this is a social worker or health professional. A full needs assessment is carried out by our nursing and social work qualified staff to build a picture of the support that is required. To help us build this picture we often use information gathered within other assessments such as Person Centred Plans; this is subject to permission being granted.
Building the Package of Care and/or Support
Our next step is to produce a written proposal of service detailing the exact care and or support required as agreed during the assessment process. We would proviude exact details of the care and support tasks to be delivered, the times and days on which these will be delivered and the cost of such a service. At this stage we are alsoable to advise you on when we would be able to commence such a service.
Costs of our service can be covered in a combination of many ways including Social Care & Health funding, Health Authority funding, Independent Living Fund, Direct Payments and/or an individual's own finances.
Care Pathways Exit Policy
There may be occasions when a user of our service no longer requires our service. This can be for a number of reasons from achieving independence to our services no longer being appropriate. In all cases, Future Home Care are committed to working alongside the service user, family and professionals to work towards a meaningful solution within agreed timescales.This is particularly important no occasions when the service being offered to the user no longer meets his/her needs. Future Home Care will endeavour to signpost service users into other services when appropriate.
