Referral
Referrals are usually made by the individual’s social worker but any body is able to make a referral including GP, community nurse, health professional, advocate, family member or the individual can refer him/herself. A simple referral form can sent to you in the post, via fax or on email, please request a form using the contact details within this pack.
Assessment of Need
Future Home Care will complete a full assessment of need and (if required) a Health Action Plan Assessment. This assessment will be carried out by our DIPSw and RNLD qualified managers.
The assessments will also take in to account previous assessments and reports carried out by other professionals. Our managers will need to work closely with all relevant professionals and carers during the assessment process to ensure all relevant information is captured and that such information is accurate and up to date.
Care Proposal/s
Once the assessment is completed FHC will produce and submit an in depth Care Proposal to the commissioning body and clinical team. This proposal will cover the following: -
- A description of the service that we will deliver including details of the individuals needs and how we propose to meet such needs.
- A description of the perceived risks posed by delivering our service and initial risk management strategies.
- A breakdown of the days and times on which we would deliver our service.
- A detailed breakdown of the cost of delivering the package including hourly rate and sleep-in/ waking night costs.
- Details of timescales of when the package can be set up and implemented.
- A description of proposed savings that may be made in regards to reduction of support as the individual gains skills and confidence to become more independent (including timescales and actual cost savings).
FHC will then await a decision from commissioners as to whether our proposal will be funded (this process is subject to local variations in systems and protocol).
